Other than having the applicable item(s), your character must also be max level (255/255).
The NPC can be found in the Forsaken King Room.

The amounts shown are cumulative. The example, The total amount of Valkyrie weapons required to go from Tier 0 to Tier 3 is 6.
Valkyrie weapon bonuses
